A decade of care – Beaumont marks 10 years of FITT (Frailty Intervention Therapy Team) improving outcomes for older adults
 

Beaumont Hospital FITT Team recently marked the 10th anniversary of its Frailty Intervention Therapy Team (FITT), a decade of pioneering, person-centred care for older adults in our community living with frailty.

Established in 2015, FITT was created to provide early assessment and intervention for frail and older patients presenting to the Emergency Department and aims to ensure older people access the right services and appropriate care pathways.

Over the past decade, the team has transformed how Beaumont supports older patients, helping them maintain independence, avoid unnecessary hospital admissions, and return home safely whenever possible.  FITT has been running in Beaumont Hospital Emergency Department (ED) since 21st of September, 2015.  

The multidisciplinary team includes physiotherapists, occupational therapists, medical social workers, Speech and language Therapists, Dieticians, Advanced  nurse specialists, and geriatricians, all working collaboratively to deliver holistic, evidence-based care.

Since its inception, FITT has had a 37% increase in patients referred to their service  (2020 versus 2024 ) and supported thousands of patients in the Emergency Department and Acute Medical Assessment Unit, significantly reducing admissions and improving functional outcomes. The service has also influenced frailty care models across Ireland, serving as a blueprint for similar initiatives nationwide.

The success of FITT is a testament to the dedication and collaboration of the entire multidisciplinary team. Ten years on, their work continues to make a real difference in the lives of our most vulnerable patients.

As Beaumont Hospital reflects on this milestone, the FITT team remains committed to continuing its mission — innovating, educating, and delivering compassionate care for older adults well into the next decade.
