Homecare is a type of care which allows people aged over 65 to receive care in the comfort of their own home. Beaumont Care in the Home Service was established in April 2022 as a result of high volume of patients awaiting home care packages to facilitate their discharge from Hospital. This service was developed to:
Patients will receive regular visits based on an assessment of their needs. The home visits are carried out by a fully trained healthcare assistant who can support them from personal care to household tasks and meal preparation. Beaumont Care in the Home cover a seven day service with a max call of 3 calls per day depending on the common summary assessment report (CSAR) . We are available from Mon to Sun) from 8am to 8pm.
This service provides help and support with everyday tasks such as:
• Personal care and hygiene
• Getting up and going to bed
• Prompting Medications from blister packs
• Promotion of Independence
• Preparing meals and helping at meal times
• Collecting shopping
• Light duties such as laundry and light housework
We accept patient referrals from patient flow, physiotherapy, occupational therapy, medical social workers or from our care of the elderly team of doctors and nurses. All patients must have an in person assessment prior to being accepted to the service
